| BREEDER'S
CERTIFICATE |

There is no fee for
this certificate. This is to be filled out only if you
breed your mare to an outside stallion. The certificate
is to be filled out by the stallion owner with the name
of the stallion and the dates the mare was bred. This
certificate is to be sent in with the application to register
the horse and stays in the horses file. |

This
is a one time fee of $25.00. A Prefix Name is a name used
before a horses name which will be used for all horses
that you register. No one else can use this name unless
they have written permission from you. |
| HORSE
TRANSFER REPORT |
| When
a horse is sold the Certificate of Registration along
with a filled out Transfer form is to be sent into the
office of PDR. The Certificate of Registration will be
up dated with the new owners name and returned to the
new owner |
